PARENTAL CONSENT REQUIRED FOR UNDER 18s TO ATTEND YOUTH PROGRAMME
Due to the fact that we will have to travel by coach to Youth Meetings
this year (as they are being held at Waterfront Community Church, outwith
Swansea University grounds) we require parental consent for all
under 18s who wish to attend the youth meetings.
PRE-REGISTER NOW!
We encourage you to pre-register for the Youth Programme before you come
to Swansea this year. The cost for registration for the week is £13.
Please send your completed form along with your cheque, payable to YOUTH
ABLAZE UK to 6 Dovecot Crescent, Dysart, Kirkcaldy, Fife, KY1 2SJ. Don't
forget to get your parents to sign your form before you send it in! (for
under 18s only)
This year's YouthAblazeUK t-shirts are limited. If you send in your pre-registration
form and circle which size of t-shirt you would prefer, we will do our
best to 'reserve' you a new t-shirt in the right size. T-shirts will be
available in a choice of SMALL, MEDIUM, LARGE or EXTRA LARGE.
